I understand that youth basketball involves inherent risks of injury and voluntarily permits my child to participate. I assume these risks and release and hold harmless the City of Margaret, Alabama, its officials, employees, coaches, volunteers, and representatives from claims arising from participation to the extent permitted by law. In an emergency, I authorize appropriate medical treatment if I cannot be reached and accept responsibility for related expenses. I have disclosed relevant medical conditions or special needs and agree to follow City rules regarding conduct and sportsmanship.
